Несмотря на то, что в соответствии с последними изменениями в российском законодательстве о том, что полис и договор могут быть одним документом, они могут быть и разными документами - отдельно договор и отдельно полис.

Igor, That's a bit of a misconception on your part.
You have "private health insurance" (as opposed to public -- "mandatory" in Russia). But "private" does not mean "individual" here because:
There is also "private INDIVIDUAL health insurance" and "private GROUP health insurance". So you have индивидуальное vs груповое страхование (individual vs. group), meaning that private health insurance can be either individual or group (not to mention other varieties).
As for "voluntary medical insurance", please see my previous note.

Still, I can hide my answer if you are 100% certain and insist that I do that. Just trying to help here, not fight.

Alex Как я понимаю, речь идет не о страховании в США или Великобритании, а в России. И справедливости ради надо сказать, что private health insurance Частное (индивидуальное) медицинское страхование - это одна из форм добровольного медицинского страхования, осуществляемого частными страховыми компаниями. Вот собствнно и все. Поэтому здесь более общим (и правильным) является термин voluntary medical insurance.

To Katerina, Thanks for raising this issue. I must note that "voluntary health insurance" is used commonly as opposed to "voluntary medical insurance". I'm not making this up, just speaking from experience. As I mentioned previously, I am not a google fan, but I suspect that if you google "voluntary medical insurance" and limit your search results to the UK or US, you will get few results. I would also like to direct the asker's attention to this Wikipedia article: http://en.wikipedia.org/wiki/Health_insurance
This should help clear up the matter of private vs public health insurance.

Health insurance (страхование здоровья) предполагает выплаты, например, при наступлении такого страхового случая, как перелом руки, т.е. сюда включено страхование от несчастных случаев. В то время как medical insurance (мед. страховка) просто покрывает расходы на лечение. Или я ошибаюсь?
И в исходном тексте речь идет именно о мед. страховке, а не о страховании здоровья.

Allow me to explain the reason for suggesting "private health insurance coverage" as opposed to "voluntary..." (which is perfectly right, by the way)... The sentence in question contains the word "обязательно..." followed by "добровольного...". These parts clash somewhat, if you ask me. Hence my suggestion of "private health insurance", which, in my experience, is used interchangeably with "voluntary health insurance". The distinction here is between "public health insurance" that is mandatory in Russia and "private health insurance" that is voluntary in Russia.
