Hello: it's the report of a patient with pulmonary hypertension.
I'm not sure whether the punctuation is faulty here (as it is in one or two places) and therefore making me read it wrongly, so here are the words around it with the punctuation as given:
"A.C.: Ritmica soplo sistolico II/VI en BED con refuerzo del segundo tono.4R dcho AP: MVC."
I understand the rest, just not the "4R dcho" - and whether e.g. it belongs to the previous sentence as shown.

Rita you are totally wrong. Please review some literature about hemodynamics. Regarding a murmur you both my be right nevertheless as I wrote (S4) will never appear on a clinical history as sound 4 because nomenclature reasons, before extend an opinion review your notes and get more real medical feedback... Thanks for your observation.

: Los ruidos cardíacos (heart sounds) se producen por el cierre de las válvulas cardíacas, y los soplos (murmurs) por el flujo de la sangre a través de una válvula enferma o de un orificio anormal/soy cardióloga, conozco el tema.
