This is part of a list of attachments to a funding application from a Colombian education establishment (after school provision). It is in response to the form's request to attach the establishment's monitoring plan. I am wondering if it is a "dummy sign in/sign out register", which may be used instead of a real register for confidentiality reasons. Any thoughts?
