My try:

I, The undersigned, (Name and last name), Director of the Judicial Investigation Office, as set forth in Law No. 69, issued this 27 day of December 2007, and at the request of the interested party:

CERTIFY
That JUAN PEREZ with XXXX citizenship, and travel document No. 1254635, as disclosed by the Criminal Records and Personal Identification Bureau concerning individual arrests, he has not been penalized for misdemeanors or any other ordinary crime.

Valid for 90 days

I think that one option could be "as disclosed by" another option could be "pursuant to information contained in..."

Anyway, thank you in advance for all your translation suggestions.

Saludos

as recorded in

I also like "as disclosed by"... but the usual translation of "constar" is record/register, which are the first options that sprang to mind
Example sentence:

...as recorded in the Criminal Records Office, it was not confined to aggressive acts...

according to the records of

I think this is the most widely used phrase in this context.

By the way, you should delete the 'he' in 'he has not been' as it should read 'That JUAN PEREZ...has not been penalized' and not 'JUAN PEREZ...he has not been penalized'.
Peer comment(s):

agree Robert Carter : Even simply "according to" would be sufficient.
