It's the header of a certificate issued by the public registry, but it lacks a more seamless and coherent structure.

1) Application is the right term here.

Toni If 'entrada' means 'reference', as I suspect it does, a full stop would be wrong. I also don't agree that 'request' and 'application' are two different things.
