This certificate has been issued for use for a young student's sports activities. I am unsure of how this phrase is structured, so what it means. Is it regarding supporting documentation as proof of health status for the sports activity of the child?

This is found in the footnotes of the certificate:

Adverinta medicala poate fi eliberata de medicul de familie la efectuarea examenelor anuale de bilant ale prescolarilor si elevilor, eliberarea acesteia reprezentand activitate de suport, conform prevederilor ordinului ministrului sanatatii .....etc

I assume the 'support or back-up activity' is the act of making a record. Otherwise, the issuing oif such certificate doesn't make much sense outside of Romania or Moldova aka Moldavia.
