We describe herein a technique for locating the ciliary sulcus for suturing a posterior chamber intraocular lens. Transscleral illumination with a light-guide fiber placed underneath the iris reveals the ciliary sulcus as a bright area near the limbus, and it allows for a straight needle with 10-0 polypropylene fiber to be easily inserted through the sulcus after making two pairs of radial scleral incisions to fixate the 10-0 polypropylene fiber. We think that this procedure can minimize complications of suturing a posterior chamber intraocular lens and shorten the operation time.

Меридиональные волокна (мышца Брюкке) прилегают непосредственно к склере и крепятся к внутренней части лимба, частично вплетаются в трабекулярную сеть. При сокращении мышцы Брюкке происходит перемещение цилиарной мышцы вперед. Мышца Брюкке участвует в фокусировке на ближних предметах, её деятельность необходима для процесса аккомодации. Не имеет такого большого значения, как мышца Мюллера. Кроме того, сокращение и расслабление меридиональных волокон вызывает увеличение и уменьшение размеров пор трабекулярной сети, а соответственно, изменяет и скорость оттока водянистой влаги в канал Шлемма.
Радиальные волокна (мышца Иванова) отходят от склеральной шпоры в сторону цилиарных отростков. Как и мышца Брюкке, обеспечивает дезаккомодацию.
Циркулярные волокна (мышца Мюллера) расположены во внутренней части цилиарной мышцы. При их сокращении сужается внутреннее пространство, натяжение волокон цинновой связки ослабляется, и эластичный хрусталик приобретает более сферическую форму. Изменение кривизны хрусталика приводит к изменению его оптической силы и перемещению фокуса на близкие предметы. Таким образом осуществляется процесс аккомодации.

Helmholtz (1856) proved that accommodation is accomplished by the change of the power of the crystalline lens associated with the active action of the ciliary body.

2. Аккомодация – способность глаза фокусировать изображение на сетчатке изображение от предметов, находящихся ближе дальнейшей точки ясного видения.
Стимулом к включению аккомодации является появление на сетчатке нечеткого изображения.
Механизм: у человека функция аккомодации выполняется цилиарной мышцей, цинновой связкой и хрусталиков, путем пассивного изменения его формы. Механизм аккомодации заключается в сокращении циркулярных волокон цилиарной мышцы, вследствие чего расслабляется циннова связка, сумка хрусталика. Хрусталик, в силу своих эластических свойств и стремления принять шаровидную форму, становится более выпуклым (возрастает его преломляющая сила), что позволяет человеку видеть предметы, находящиеся на близком расстоянии.

The anatomic zone for optimal placement of intraocular lens (IOL) implantation is within the capsular bag. This provides the greatest IOL stability and places the IOL closer to the nodal point of the original crystalline lens, resulting in better image resolution.1 This position also keeps the foreign lens material away from the anterior chamber, thus preventing complications involving the endothelium and anterior chamber angle.1 However, when the IOL cannot be securely placed within the capsular bag, an alternative positioning and fixation is required. In cases of posterior capsule rupture, zonular laxity, lens subluxation with absent capsule, or planned piggyback IOL placement, IOL placement in the ciliary sulcus is preferred.1,2 Surgeons must be familiar with the anatomy and indications for IOL placement in this space in order to maximize chances of visual rehabilitation and decrease risk of vision threatening complications.
Ciliary sulcus anatomy and measurements have been reported in cadaver eyes and more recently in the living eye using endoscopic imaging and ultrasound biomicroscopy (UBM). The exact sulcus-to-sulcus distance cannot be measured, but it is estimated to be approximately 11.0–12.5 mm based on anatomic findings of the ciliary body, corneal diameter, and white-to-white measurement.3

Secondary intraocular lens (IOL) implantation is a common treatment for pediatric aphakia. 1–3 In
short- to midterm follow-up of 12 to 60 months, the incidence of adverse events (AEs) after IOL implantation
in pediatric aphakia has been reported to be as high as 19.5% to 28.5%, affecting the prognosis and visual function
