To whom it may concern:
Our office has recently received confirmation that Dr. X will be a University of Alberta Resident
effective April 2023. As part of their appointment, Dr. X will be working in Alberta Health Services
(AHS) facilities.
Under the Protection for Persons in Care Act, Alberta Health Services has a legal obligation mandated by the
government to ensure that all parties engaged in the delivery of care or support services undergo both:
1. a police information check, sometimes called a criminal records verification or a criminal records
check; and
2. a vulnerable sector search, sometimes called a violent offenders search or a sexual offenders
search.
We have formally required that Dr. X provide this document to our office prior to their start date;
please accommodate this Resident with this request
