Other medical professionals' names are followed by le, lf, aa, ae, ch - not sure what any of them mean

Thanks for the suggestion! But I don't think that can be it as the same person who's initials are BW appears twice followed by both GB and .ip

username? Could these be the nurse's username in the computerised medical records system? Often the username is 2-4 letters (taken from the person's first and last name) plus 2 numbers.
