In a report on an MRI of the lumbar spine (in asterisks) - rest of para for context:

"Rezidivierendes unklares Lumbalsyndrom bei beiderseitigem Hüftimpingement nach stattgehabter lumbaler Nukleoplastie (2/2021) mit postoperativ persistierenden Lumbalgien, muskulären Verspannungen und pseudoradikulären Lumboischialgien mit beiderseitiger Ausstrahlung in die lateroposteriore Beckenregion und intermittierend bis in die Leisten bei deutlich belastungs- und positionsabhängig *betonbarer* Beschwerdesymptomatik im Sitzen. Zusätzliche Muskelkrämpfe in der linken Oberschenkelregion. I have seen highlight and emphasize in PROZ, but not many hits in the internet for "highlightable" - I guess this means that this can be highlighted on the MRI scan?

I'd rephrase a bit and avoid translating betont literally. It simply means that the symptoms get worse/are more pronounced on exertion and postural changes.

clearly attributable / distinct

distinct or clearly attributable symptoms, in other words not vague. They can clearly be attributed to an area/condition

In this case, the pain or condition that the symptoms can be attributed to is activity-and position-related (belastungs- und positionsabhängig)
