Japanese to English Other Law (general) Child adoption

This may not be of immediate importance, but does anyone know the meaning of the above 5 discrete abbreviations that are found in the left-hand column of Japanese adoption notifications （養子縁組届)? They are listed next to the subheadings 養子女、親父母、使者. Some of the boxes next to these abbreviations will be check-marked, but I'm not sure what, if any, obligations they entail.
