I can only but guess your original might be talking of an acute ST-elevation myocardial infarction (a STEMI) -un ACS "CESST"; acute coronary syndrome "con elevación del (segmento) ST" (?).
As for postrombolisis (post-thrombolysis) it might well refer to the STEMI itself (a post-thrombolysis STEMI) rather than to the patient's clinical history (ischemic chardiomyopathy and type II Diabetes).
So, the original might be trying to convey this:
Post-thrombolysis STEMI in a patient with ischemic cardiomyopathy and type II diabetes.
Anyhow, it might be helpful to read the original Spanish as is.
