Here there is a quite clear definition of advanced directives and different types of these:

The most common types of advance directives are the living will and the durable power of attorney for health care (sometimes known as the medical power of attorney).

And by the way, ADRTs (advanced directives to refuse treatment) are but a type of advanced directives; ADs are not necessarily "negative" instructions (i.e. to refuse CPR, to refuse hospitalization and the like).

Advance Decision / Living Will

An Advance Decision (also known as a Living Will, Health Care Directive or Personal Directive) is quite different from an advance statement

"An advance decision to refuse treatment lets you, while you have mental
capacity, choose and explain which medical treatments you do not want
doctors to give you, if a time comes when you lack capacity and cannot
make the decision or communicate your wishes. If you want to refuse
life-sustaining treatment, you must put this in writing, sign and date it in
presence of a witness."

It is more than an advance statement which "allows you to make general statements, describing your wishes and preferences about future care should you be unable to make or communicate a decision or express your preferences
at the time."

US & UK: Advance Medical Consent Form

a pre-operation form doesn't look to me like a Living Will, an Enduring or Lasting Power of Attorney and consent form works Transatlamtically.

Having drafted consent forms myself, albeit many years ago, albeit I am a bit surprised only David H. has mentioned such in his reference.
