The phrase is in a dental research paper written in Argentina.
"Criterios de inclusión: Individuos que concurren a un centro de atención odontológica por demanda espontánea"

Although the direct translation "spontaneous demand" appears on google in medical contexts, they all seem to be papers from Spanish or Portuguese-speaking countries.
Does anyone know what the phrase might be in English?
