First Break is the act of breaking the integrity of process piping or equipment that might contain
toxic and hazardous products. It is a potential planned loss of containment where there is
potential for trapped pressure, or exposure to a toxic commodity or a commodity has not yet
determined to be toxic or non-toxic.
Operations typically complete First Break as operators normally drain/depressurize equipment
prior to LOTO.
Maintenance and Construction also typically complete First Break (e.g. breaking a flange or
opening a valve on a line that contains potential for H2S, Benzene or any other respirable
harmful substances.)
A vent or drain can be the first break on a system and if a vent or drain valve test has proven
that first break is free of hazardous pressure or fumes then any flange following the first break
would be considered second break and can be done with a proper job safety analysis having
been performed. Note: A second break must be treated as a first break if there is an equal risk
of exposure or trapped pressure potential.

Речь идет об остановке производства на неком химзаводе/НПЗ для какого-то ТО и прочих работ. Естественно, при доступе внутрь оборудования возникают риски для сотрудников

Почитал цитату в ответе Алексея и должен изменить свой ответ. Это не останов производства, а именно первая разборка/вскрытие оборудования (с потерей его герметичности) для разных работ ТО уже после останова.
