No, you don't have to. I did that to explain it. It is not the time but a positioning/a direction, as a point of reference

You are talking of a positioning, not the time. ' on a 7 o'clock position/direction'. 'on' the tumor, in this case. Pilots/astronomers use it, amongst others. It is just a simile.
