Sworn declaration versus affidavit A sworn declaration (also called a sworn statement or a statement under penalty of perjury) is a document that recites facts pertinent to a legal proceeding. It is very similar to an affidavit, but unlike an affidavit, it is not witnessed and sealed by an official such as a notary public.

I'm not sure if there's any difference, but it seems "affidavit" and "sworn statement" can be used interchangeably.
I'd be interested to see what the experts have to say about this.

disagree AllegroTrans : An affidavit, being a creation of common law countries, is NOT the same as a sworn statement; have a look at the sample wording and you will see the difference, no expert needed

I don't know what country this is for, but I'd believe that they wanted the statement to be sworn before a notary public. Wouldn't that make it an affidavit then? IMO, it wouldn't hurt to hear other people's opinions
