I'm not sure if there's any difference, but it seems "affidavit" and "sworn statement" can be used interchangeably.

: An affidavit, being a creation of common law countries, is NOT the same as a sworn statement; have a look at the sample wording and you will see the difference, no expert needed

I don't know what country this is for, but I'd believe that they wanted the statement to be sworn before a notary public. Wouldn't that make it an affidavit then? IMO, it wouldn't hurt to hear other people's opinions
