disagree Gintautas Kaminskas : "Document to be signed" is just a clumsier (wordier) way of saying "signature document". In my 30-year career in the Australian Public Service I heard the phrase "signature document" dozens of times. I don't imagine it would be any different in UK.

It's a simple and accurate way to describe the type of document which is referred to in the question. I have never seen 'signature document' used in any context in the US or the UK.
