I am translating an entire legal document which is a תשובה לתגובה. I only have this document and I don't know the background. Is this expression translated as 'reply to an answer' (I think this is it) or 'reply to a response'? Or could it be either depending on what they are replying to?

This will generally be in reply to the response by the respondents to a motion of some sort by the appellant/claimant (motion to approve a class action or motion to dismiss, for example). If so then it will be "reply to the response". If you are dealing with a specific case then even out of context you might assume that it is "reply to the response" rather than "reply to a response".
