Country of origin might clear that up. Referido has always been referral in the countries I've worked, but the Castillian language has so many microclimates.

Discussion
I find lots of examples in Linguee of "referido" = "referral" ... for instance:
" ... por favor notifique al departamento HR sometiendo la forma de referido disponible en la oficina HR " --> " ... please notify the HR department by submission of the referral form available in the HR office."
I have one small doubt about this, and whether it might not mean infection "report" form, based on this DRAE entry: referir: 1. tr. Dar a conocer, de palabra o por escrito, un hecho verdadero o ficticio.
Most likely, you can trust Jason's suggestion ... he knows what he's talking about.
For context ... to go even farther ... possibly mentioning the country where the text comes from might be useful. at least in some situations.
