Russian to English Other Education / Pedagogy university
This is in relation to the form which Russian university teacher's write the student's examination results on (particularly pertaining to exchange students) and comes in the form of a single sheet with a table with all the courses taken are listed and scores from each exam are handwritten and signed by the teacher for that course.

When I was a university student it was in the form of a book rather than a sheet.

And it contained the results of not only scored but also pass/fail exams - year after year.

Based thereon an academic transcript was issued (with the final/last score on each discipline's course that could last two-three-four years, e.g., English Language in my case,) which transcript became an insert attached to the 'university diploma'.
