"al Duque" is sometimes used as part of an idiom meaning "both ways".
querer ir por atún y ver al duque, for example, means "to want to have it both ways".
Reading the recipe we see that this meal is cooked twice in 2 different ways.
As Charles say, better leave it as "al duque".

Benalmádena ... has several streets named after one Duke or another, and the dish may refer to one of them.

@ Andrew I should contact the "Alvorada" if you have the possibility and ask for a description of the dish. You could also simply translate it as "cod au gratin 'al duque'". The british consumer can always ask the waiter, what the dish is like. ;)

No probs TBH I don't have much of a culinary clue and it might well be a local dish. Like wines (particularly the good ones) I concentrate on consuming rather than remembering names ;@)

Having said that, it is fairly typical in bars near me to include plates with names of local 'celebrities', historical characters, etc.
My local peña Sevillista has "Kanoute, Navas, Adriano, etc. mondatitos" ;@)
Obviously less typical in a good restaurant.
