I was considering 'notarised deed of constitution' but I think this only refers to companies. Anybody got any ideas? Going into British English.

notarised instrument of formalisation

A mouthful, I admit, but I don't think we have anything remotely comparable in the UK. It would not be a deed, strictly speaking, because that's for transfer of property. The general term for an escritura is really an instrument.

There's a thing in Texas called a declaration of marriage for common-law couples that is somewhat comparable to this, but I wouldn't use it here, because it doesn't fit very well in your context. I think you need a verb for "constituir" and a corresponding noun for "constitución", and I suggest that formalise/formalisation would fit the context here. Anyway, it's a suggestion.
