b) Que dichos objetivos se han orientado hacia el cuidado de la salud a través de la *medicina asistencia!*, y han comprendido también actividades de docencia e investigación en el entendimiento que la buena praxis médica requiere de actividades académicas e investigativas que ayuden a desarrollarla y la complementen;

I've made my research, but I'm not sure:
Cuidado de la salud: health care.
Medicina asistencial: medicine assistance.

medical assistance

As I see it, the difference here is simply between healthcare/medical assistance (atención médica, en general), i.e. working with patients on the one hand, and the "actividades de docencia e investigación" on the other.

So, it's not really a question of primary vs secondary care, but practice versus theory/research.

This refers to the kind of treatment you would receive from a primary care physician, which is usually the first health professional to examine a patient and who recommends secondary care physicians.
