Los enlaces que pegaste confirman que "factura en la sombra" y "shadow bill" son la misma cosa. Además confundes aquí tres terminos distintos.

En relación al resto no creo que esperabas una respuesta. Si te sientes ofendido por no sé que razón siempre puedes pedir ayuda a un moderador. Así puesto al público parece poco profesional.
El término lo entiendo perfectamente. Es más, la Sra. Algazi parece compartir mi punto de vista de que se trata de una réplica de la factura para la HMO, que es enviada a Medicare en determinadas situaciones bien específicas. Nada que ver con informar al paciente sobre el coste de su tratamiento.

Lo que sí que no entiendo es la agresividad latente de tu comentario. Otros traductores podrían haber criticado duramente tu error de pensar que shadow bill y "factura en la sombra" se refieren al mismo concepto, basándote simplemente en el parecido léxico, y sin embargo nadie lo ha hecho.

Una parte importante de ejercer la traducción, igual que cualquier otra profesión, es saber aceptar las críticas sin ponerse a la defensiva. Por supuesto, estás en tu pleno derecho de creer tener razón, pero de allí a insinuar que otro traductor no entiende textos sencillos en sus idiomas maternos hay un buen trecho.

En fin, disculpas a Alfredo y a los demás traductores por esta parrafada, pero seguro que entendéis mi punto de vista.

¿Envía a Medicare copias de las facturas que emite para las HMO?

The concept of shadow billing in health care is described here:

Since there is no direct translation, I'd go for a descriptive one. "Health Maintenance Organization" is sometimes translated as Organización de Mantenimiento de Salud, but it may be useful to leave the original English acronym. It's your call.

disagree Katarzyna Müller : si no entiendes el término, ni lo que pone en el enlace que pegaste, por lo menos no "corrijas" a otros, "shadow bill" en español se llama "factura en sombra". Un saludo.

agree FlyHi : None of the references mentions letting patients know how much their treatment cost. I think more information from the asker (e.g. context, country of origin of the text) could help us find the right translation. As things stand, this is the safest bet.
