Diagnosis: Left distal tibia/medial malleolus free gracilis myocutaneous flap

The patient sustained a nasty oblique fracture of the distal bone of his tibia and fibula involving his medial malleolus. This was reduced with open reduction and internal fixation and unfortunately he developed a skin defect following the operation. He initially underwent a local perforator based fasciocutaneous flap to the defect on his medial distal left calf. Unfortunately this developed some thick staining of the skin and had to be divided. This was debrided down to metal again and a VAC pump applied and he subsequently went on to have a free myofasciocutaneous gracilis flap. The metalwork is still in situ and the patient is now walking freely without any aids.
