settlementには色々な意味がありますが、上記の文を読む限りでは、決算のことではないかと思います。2007 Comprehensive Spending Reviewは2008-2009と2009-10年の会計年度を含む部門のための決算を発表し、NHSの全体的な資金がこれからの3年の間に年4％増加させる計画を立てている、という内容と解釈しました。
