Er betrachtet das Organ, tastet es vorsichtig ab und bereitet sich so auf die anstehende Operation vor. "Das Herz ist vernarbt, der Eingriff kompliziert", erklärt der Chefarzt vom Herzzentrum Brandenburg. Das originalgetreue Modell aus weichem Kunststoff hilft dem Spezialisten. "Ich kann die Herzhöhlen begreifen und besser nachvollziehen, was mich während der Operation erwartet", sagt er.
Ein solches Organ kostet zwar einige tausend Euro, stellt aber in schwierigen Fällen eine große Hilfe dar. "Mediziner können an den 3D-Modellen sägen, schrauben und fräsen. In China rekonstruierten Ärzte kürzlich das von Geburt an stark deformierte Gesicht und den Schädel eines kleinen Mädchens, und zwar mit Hilfe eines Modells aus dem 3D-Drucker. Bevor das Mädchen operiert wurde, konnten die Fachleute daran die komplizierte Prozedur proben.
Die Idee, dass eine Maschine per Knopfdruck maßgeschneiderte Produkte herstellt, begeistert Laien wie Fachleute gleichermaßen. Die additive Fertigung - auch industrieller 3D-Druck genannt - gilt als nächste große technische Revolution. Ob Gelenkimplantate, künstliche Stents für Luftröhren, Komplettprothesen oder Zahnersatz - die Liste der medizinischen Produkte aus den 3D-Druckern wächst stetig. "Fast jede erdenkliche, mit einem 3D-CAD-Programm konstruierbare Form oder Geometrie lässt sich additiv fertigen", erklärt Hund. Es gibt nahezu keinerlei Einschränkungen - auch nicht bei der Herstellung von Gelenken oder hohlen Strukturen. "Selbst kleinste anatomische Strukturen lassen sich nachbilden", sagt er.
Customized Medical Devices: Langzeiterfahrung fehlt
Bis eine medizinische Maßanfertigung - auch unter der Bezeichnung Customized Medical Device bekannt - in den Körper des Menschen gelangt, hat sie jedoch einen weiten Weg hinter sich. Die Produkte müssen aufwändige Zertifizierungsverfahren bestehen. "Der Medizinmarkt ist streng reguliert", sagt Martin Bullemer, Business Development Manager Medical beim Technologieunternehmen EOS. Sterilität, ausgezeichnete Verträglichkeit sowie lange Haltbarkeit stellen ein absolutes Muss dar. Die Anbieter müssen die Verantwortlichen in den Krankenhäusern für ihre Produkte gewinnen. Und selbst wenn die Mediziner und Prüfer überzeugt wurden, bedeutet das nicht automatisch einen abschließenden Erfolg. "Wir haben noch keine Langzeiterfahrungen mit den Produkten", betont Professor Dr. Christian Lüring vom Klinikum Dortmund. Wenn er eine Operation am Knie durchführt ist die Wahrscheinlichkeit groß, dass ein Knie-Implantat aus dem 3D-Drucker zum Einsatz kommt. "Etwa 20 bis 25 Prozent unserer Patienten entscheiden sich dafür", berichtet der Direktor der Orthopädischen Klinik. Ob die 3D-Alternativen wirklich besser seien als die konventionellen Implantate wisse man in etwa fünf Jahren, meint er.
Die neue Technologie wartet jedoch mit handfesten Vorteilen auf: "Das Implantat wird wie ein Maßanzug auf das Knie geschneidert, die Knochen werden besser abgedichtet. Das Ersatzteil kann dadurch gut anwachsen", erklärt Lüring. Nicht jeder Patient benötigt ein maßgeschneidertes Ersatzteil fürs Knie. Die Stücke eignen sich besonders für Menschen, bei denen es Probleme mit den Standardprodukten geben könnte. "Dazu gehören zum Beispiel sehr große und sehr kleine Patienten", sagt Lüring. In begründeten Fällen zahlt die Krankenkasse ein per 3D-Druck gefertigtes Implantat, das etwa doppelt so viel kostet wie ein herkömmliches. Lüring gehört sicher zu den Pionieren in diesem Bereich, bislang nutzen erst einzelne Kliniken die modernen Implantate.
Etabliert haben sich in den Operationssälen dieser Welt spezielle Einwegprodukte, die aus dem 3D-Drucker stammen und patientenspezifisch angewendet werden - zum Beispiel sogenannte Bohrschablonen. "Von ungefähr 1,2 Millionen Knie-Operationen weltweit nutzen Mediziner bei etwa einem Drittel Bohrschablonen, die per 3D-Druck gefertigt wurden", sagt Bullemer. Die individuell für den Patienten angefertigten Stücke würden viel Planungsarbeit während der OP ersparen.
Die 3D-Printing-Revolution in der Zahnmedizin

Zahntechniker erheben die notwendigen Daten durch Scannen des Kiefers und können innerhalb von ungefähr 24 Stunden bis zu 450 individuelle Dentalkronen und Brücken herstellen. "Ansonsten schafft ein Zahntechniker vielleicht 15 bis 20 Einheiten am Tag", sagt Martin Bullemer. Er zeigt sich zuversichtlich, dass sich die Technologie auch in anderen Bereichen der Medizin durchsetzt. "Wenn in einigen Jahren die Langzeitstudien vorliegen und es noch schnellere Maschinen gibt, wird die Fertigungsmethode noch sehr viel bedeutender", ist er sicher. Der Patient liefert die Daten und erhält eine auf ihn zugeschnittene Therapie, die wirtschaftlich und medizinisch Sinn macht, so die Vision.

"Bei diesen Versuchen setzt man aufwändige Einrichtungen ein, die eher einer komplexen, computergesteuerten Laboranordnung als einem 3D-Drucker ähneln", beschreibt Professor Hartmut Schwandt von der Technischen Universität Berlin die Bemühungen. Er experimentiert mit seinem Team an einer Zwischenlösung. "Wir arbeiten daran, ein Herzklappengerüst mit dem 3D-Drucker zu erstellen, das am Deutschen Herzzentrum mit menschlichen Zellen besiedelt wird", erklärt der Wissenschaftler. "Approximately 20 to 25 percent of our patients opt for it," reports the director of the Orthopaedic Clinic. Whether 3D alternatives are really better than conventional implants, we won't actually know for about another five years, he says.
However, the new technology offers tangible advantages: "The implant is cut and sectioned for the knee like a tailor-made suit, the bones are sealed off better. This enables the prosthesis to adhere well," explains Lüring. Not every patient needs a tailor-made spare part for the knee. Den 200. Geburtstag des Reichsgründers Otto von Bismarck feiern die Deutschen nicht. Der einst verehrte und verklärte Politiker scheint heute weit weg. Erinnerung an einen schwierigen Staatsmann von Christian F. Trippe.
Allen seinen Biographen ist das Vielschichtige seiner Persönlichkeit aufgefallen. Alle betonen das Widersprüchliche, das sich durch Charakter, Taten und Texte des Mannes zieht, der als "Eiserner Kanzler" schon zu Lebzeiten zum Mythos der Deutschen wurde. In seiner Brust wohnten nicht nur zwei Seelen, sondern da ginge es zu wie in einer Republik, hat er einmal gesagt. Ein Mann mit (zu) vielen Eigenschaften, mit überbordender politischer Energie, der schon zu Lebzeiten polarisierte wie wenige vor und nach ihm.
Das politische Ziel, die zersplitterten deutschen Territorien unter Führung des Hauses Hohenzollern zu einigen, beschrieb er in der Sprache der Gutsbesitzer: „Setzen wir Deutschland in den Sattel. Reiten wird es schon können.“ Es konnte nicht. Das Reich, das er schuf, überdauerte seinen Tod nur um 20 Jahre; in einer faschistisch entstellten Variante kam es noch einmal zurück. 1945 war Schluss - mit dem Reich und mit Preußen, mit dem Reichsgedanken und mit dem Obrigkeitsstaat.
Als Denkmal präsent bis heute
Denkmäler und nach ihm benannte Türme aber blieben stehen bis heute, tausende Straßen, Plätze und Alleen in Deutschland tragen weiterhin seinen Namen. Der Kult, der schon zu Lebzeiten begann, verblasste erst mit der Niederlage im Zweiten Weltkrieg; mit der Wiedervereinigung aber wurde er nicht wieder belebt. Es ist still geworden um das 19. Jahrhundert, dessen zweite Hälfte er so sehr geprägt hatte.
Das 1871 von ihm gegründete Deutsche Reich hielt er für saturiert, auch ohne Kolonien in Übersee. Und so entdecken ihn dieser Tage ausgerechnet die deutschen Sozialdemokraten ein wenig neu—den maßvollen und ausgleichenden Außenpolitiker, den "ehrlichen Makler", dem das Verhältnis zu Russland immer besonders wichtig war. Das entbehrt nicht einer gewissen Ironie, war er es doch, der als unerbittlicher Sozialistenfresser die SPD von Staats wegen unterdrückte. "Reichsfeinde" vermutete er in jedem Arbeitersportverein, auch in jedem katholischen Gesellenverein.
Die Katholiken schmähte er als "Ultramontane", deren Loyalität eher beim Papst in
Rom, denn beim deutschen Kaiser liege. Und denen er deswegen im "Kulturkampf" zusetzte. Unbestritten, dass mit ihm, der seine Reichstagsreden immer in preußischer Generalsuniform zu halten pflegte, ein Zug der Militanz und Unerbittlichkeit in Deutschlands Innenpolitik kam. Die Folgen dieser innenpolitischen Freund-Feind-Logik waren schlimm.
Ein Meister der geflügelten Worte
Eingängig und manchmal anstößig waren seine Worte, denen oft Flügel wuchsen: "Wir Deutsche fürchten Gott, aber sonst nichts auf der Welt" war so ein Diktum, das er übrigens auf dem Altenteil im Sachsenwald bereute, als er sah, welches Eigenleben die nassforsche Sentenz unter dem geltungssüchtigen Kaiser Wilhelm II. entwickelt hatte. Zumal sein frommer Nachsatz es nicht in den öffentlichen Gebrauch geschafft hatte: "Und die Gottesfurcht ist es schon, die uns den Frieden lieben und pflegen lässt."
Die SPD suchte er mit seinen Sozialgesetzen politisch überflüssig zu machen - und schuf die verpflichtende Kranken-, Unfall- und Rentenversicherung. Im Grunde sind sie bis heute unverändert geblieben; geboren aus der Frontstellung gegen den politischen Feind auf der Linken und um die ungeliebten Liberalen zu düpieren. Mit ihm, dem Urpreußen und Erzkonservativen, schlug die hohe Stunde des politischen Konservatismus in Deutschland.
Kein Vorbild mehr, nur noch Folklore
An seinem 200. Geburtstag leben die Deutschen gut mit und in dem Sozialstaat, den er aus politischer Verlegenheit geschaffen hat. Doch keine Partei in der Bundesrepublik hat sein Erbe angetreten, nicht einmal eine namhafte politische Strömung oder Vereinigung beruft sich auf ihn. Als Vorbild taugt er nicht mehr, denn sein Leitbild, der klassische National- und Machtstaat, hat sich überholt.
Den Deutschen ist er bestenfalls noch Bestandteil historischer Folklore. Sie kommen gut über die Runden, auch wenn sie - so wie dieser Text - seinen Namen gar nicht erwähnen.

"Let us put Germany in the saddle," was the kind of language he used in describing unifying the fragmented German territories under the leadership of the House of Hohenzollern. "It'll surely know how to ride." But it didn’t. The Reich he created exceeded his death by only 20 years. It came back, certainly, albeit as an irrevocably defaced fascist variant. 1945 was the end: the end of the Reich, the Prussians, the Reich mentality, and the authoritarian state.
A monumental presence to this day
Monuments and towers named after him remain so today. German parks, streets, and avenues numbering in the thousands still bear his name. The cult of the Iron Chancellor, which began during his lifetime, started to fade only after Germany's defeat in WWII. After the Fall of the Berlin Wall, it could never again reach its former glory. No one talks about the late 19th century in Germany anymore, so much of which he had shaped.
Founding the German Reich in 1871, already then and even without overseas colonies, he felt it saturated. Nowadays, the Social Democrats of Germany (the SPD) of all people are trying to cash in on that line of thought, choosing to see him as the truthful and balanced German diplomat, the "modest middleman" to whom Germany’s relationship with Russia was always of utmost importance. Yet this stance certainly isn't without a bit of irony, since he was the one, after all, who relentlessly repressed the SPD by any governmental means necessary. "Enemies of the Reich," he assumed of every workers' sports club and every Catholic Journeymen's Association.
He reviled the Catholics as "ultramontanes," those who preferred professing their loyalty to the Pope in Rome rather than the German Kaiser. He badgered them with a Kulturkampf or "culture war," precisely because of their misplaced loyalty. That he, always giving his German parliamentary speeches in his Prussian General’s uniform, brought a train of militancy and relentlessness into Germany's domestic politics, is undisputed. And the consequences of this domestic friend-enemy logic were dire indeed.
A master of winged words
His particular way of phrasing things, often catchy and sometimes offensive, almost always grew wings: "We Germans fear God, but nothing else in the world," was such a phrasing. He eventually came to regret it, though, while stationed in the old part of the Sachsenwald (a forest near Hamburg that was gifted to him in 1871 by Kaiser Wilhhelm I). He saw how this brash aphorism took on a life of its own under the attention seeking and validation craving Emperor Wilhelm II. He especially regretted it since his pious postscript failed to reach the public’s ears: "And it is the fear of God that makes us love and cultivate peace."
Creating obligatory health and accident insurance and pension plans through the enactment of his social laws, he tried to make the SPD politically superfluous. Today, those laws have remained practically unrevised, borne as they were out of animosity towards political enemies on the left, and the desire to pull the rug out from under the unloved liberals. As far as he, the old Prussians, and the ultra-conservatives were concerned, it was high noon for political conservatism in Germany.
But no paragon here—only folklore and myth
On his 200th birthday, Germans are living well with and in the welfare state that he created out of political embarrassment. Yet no party in Germany's federal government has officially taken up his mantle; in the established political currents and associations, there’s barely a whisper of his name. He’s not even an example of good politics anymore, since in and of itself the idea of the classic nation and power state has become obsolete. In the wake of the 20th century, Germans do what they can in making ends meet, even by, just like this article, not mentioning his name. The cult of the Iron Chancellor might ever so barely linger but, to Germans, his mythic power and influence will only ever be just that—a myth.
