Votre patient Monsieur XXXXXXX âgé de 60 ans, s’est
présentée à la consultation pour suivi de troubles du rythme
Les antécédents vous sont connus. Rappelons une Torsion
testiculaire , un WPW connu depuis l'âge de 18 ans avec
disparition de l'onde delta à l'effort, une dysthyroidie avec poussée
d'hyperthyroïdie en 2008 sur intoxication à l'iode prouvé sur
dosage urinaire, une carence en Ig A . Une atteinte méniscale au
genou droit qui s’est résolue par kiné.
Facteurs de risques cardio-vasculaires :

Monsieur XXXXX revient en contrôle.
Sur le plan cardiaque, il joue moins au Padle et au tennis.
Rappelons qu’il a arrêté le karaté depuis 5 ans. Il fait moins de
sport des suites de son travail. Il ne signale pas de dyspnée ni de
douleur thoracique mais il garde des palpitations quand il est
fatigué. Il n’a pas de vertiges ni d’OMI.
Sur le plan général, il a facilement de légers vertiges (mal de mer)
en voiture si il ne conduite pas. Il dort moins bien qu’avant se
réveillant 2 à 3 fois certaine nuit avec parfois une nycturie et des
ronflements nocturne. Il n’a pas de fatigue diurne. Il a grossi de 2
kgs.
Biologie réalisée le 21 avril 2017:
Cholestérol : 155 mg% LDL :86 mg% HDL: 71 mg%
Tryglicérides : 60 mg% Créatinine :0.95 mg% Glycémie : 93 mg%
PSA : 0.5 Acide Urique : 5.4 mg%
Doppler couleur : Normal
Pression pulmonaire estimée à 17 mmHg.
Onde E/A : 0.89 Insuffisance mitrale de grade I.
Pic Gradient aortique : 6 mmhg
Minime Insuffisance aortique de grade I.
Epreuve d’effort sur cyclo-ergomètre par paliers de 25 Watts/min:
Charge maximale atteinte : 225 Watts
Fréquence maximale atteinte : 142 bpm .
Tension artérielle au max : 230 / 110 mmHg .
Tension artérielle après plus de 3 minutes de récupération : 130 / 70
Motif d’arrêt : Fatigue des jambes.
Segment ST : Non modifié
Troubles du rythme : Rares extrasystoles supraventriculaires isoléees
Conclusion :
A l’examen clinique, son auscultation cardiaque est normale, les pouls périphériques sont palpés, l’auscultation pulmonaire est normale. On objective une tension artérielle normale au repos, à l’effort et en récupération.
Son rythme est bien régulier et son ECG de repos est normal sans trace de son WPW. A l’échographie cardiaque, on retrouve une excellente cinétique segmentaire sans dilatation des cavités. Il n’y a pas d’hypertrophie pariétale. On objective une discrète insuffisance mitrale et aortique de grade un centrale favorisée par une calcification d’une cuspide aortique. Les pressions pulmonaires sont normales.
Sa tolérance à l’effort est excellente sans modification du segment ST ni troubles du rythme.
On peut être tout à fait rassuré sur le plan cardiologique. Une biologie serait utile au début de l’année.

On a cardio level, he plays less Paddle and tennis.
As a reminder, he stopped karate 5 years ago. He does less sports because of his work. He has reported no dyspnea or chest pain, but he continues to have palpitations when he is tired. He has no vertigo or IMO.
On the general level, he easily has mild vertigo (carsickness) in the car if he is not driving. He does not sleep as well as before, waking up 2 to 3 times certain nights with occasional nocturia and nocturnal snoring. He has no daytime fatigue. He has gained 2 kgs.

Conclusion:
After clinical examination, his cardiac auscultation is normal, the peripheral pulses
are palpated, pulmonary auscultation is normal. Normal blood pressure has been measured at rest, exercise and recovery.
His rhythm is regular and his resting ECG is normal without trace of his WPW. The cardio-ultrasound shows excellent segmental kinetics without dilatation of the cavities. There is no parietal hypertrophy. A slight central grade mitral and aortic insufficiency is observed, favored by calcification of the aortic cusp. Pulmonary pressure is normal.
His tolerance to effort is excellent without modification of the ST segment nor rhythm disorders.
We can be absolutely reassured on the cardiological level. A biological exam would be useful at the beginning of the year.
