Source text - English Background—Increased thrombogenicity and smooth muscle cell proliferative response induced by the metal struts compromise the advantages of coronary stenting. The objective of this randomized, multicenter study was to assess whether a reduced strut thickness of coronary stents is associated with improved follow-up angiographic and clinical results.
Methods and Results—A total of 651 patients with coronary lesions situated in native vessels > 2.8 mm in diameter were randomly assigned to receive 1 of 2 commercially available stents of comparable design but different thickness: 326 patients to the thin-strut stent (strut thickness of 50 μm) and 325 patients to the thick-strut stent (strut thickness of 140 μm). The primary end point was the angiographic restenosis (≥ 50% diameter stenosis at follow-up angiography).
Secondary end points were the incidence of reinterventions due to restenosis-induced ischemia and the combined rate of death and myocardial infarctions at 1 year. The incidence of angiographic restenosis was 15.0% in the thin-strut group and 25.8% in the thick-strut group (relative risk, 0.58; 95% CI, 0.39 to 0.87; P50.003). Clinical restenosis was also significantly reduced, with a reintervention rate of 8.6% among thin-strut patients and 13.8% among thick-strut patients (relative risk, 0.62; 95% CI, 0.39 to 0.99; P50.03). No difference was observed in the combined 1-year rate of death and myocardial infarction.
Conclusions—The use of a thinner-strut device is associated with a significant reduction of angiographic and clinical restenosis after coronary artery stenting.
Translation - Russian Общие предпосылки: Металлические элементы каркаса стента способствуют активации тромбообразования и пролиферативного ответа гладкомышечных клеток, что может свести на нет все преимущества стентирования коронарных артерий. Цель данного рандомизированного многоцентрового исследования – установить, связано ли уменьшение толщины металлических элементов каркаса стента с улучшением ангиографических и клинических результатов стентирования.
Методы и результаты: В исследовании принимали участие пациенты (n = 651), имеющие стенозы в нативных коронарных артериях диаметром более 2,8 мм. Участники исследования были рандомизированы на получение одного из двух доступных на рынке стентов аналогичного дизайна, различающихся толщиной элементов каркаса: 326 пациентам были установлены стенты с тонкими элементами каркаса (толщина элемента каркаса 50 мкм), 325 пациентам – стенты с более толстыми элементами каркаса (толщина элемента каркаса 140 мкм). Первичной конечной точкой был ангиографический рестеноз (стеноз, суживающий просвет сосуда на 50 % и более, по данным контрольной ангиографии). Вторичные конечные точки: частота повторных вмешательств по причине ишемии, вызванной рестенозом; общая частота летальных исходов и инфарктов миокарда за 1 год (комбинированная конечная точка). Частота ангиографических рестенозов составила 15,0 % в группе стентов с тонкими элементами каркаса и 25,8 % в группе стентов с более толстыми элементами каркаса (относительный риск 0,58; 95 % доверительный интервал, 0,39–0,87; Р = 0,003). Статистически значимо уменьшалась и частота клинических рестенозов. В группе стентов с тонкими элементами каркаса повторные вмешательства проводились в 8,6 % случаев, тогда как в группе стентов с более толстыми элементами каркаса – в 13,8 % случаев (относительный риск 0,62; 95 % доверительный интервал, 0,39–0,99; Р = 0,03). The Tri-Lock® Bone Preservation Stem incorporates Gription™ fixation technology. Gription™ is designed to help provide consistent
implant seating height and additional initial stability that helps maximise the potential for long-term bony ingrowth.
Neck osteotomy
Align the neck resection guide with the long axis of the femur. This establishes the angle of resection at a proper 50˚ from the femoral axis. Determine the resection level by aligning the top of the guide with the tip of the greater trochanter or by measuring a pre-operatively determined distance above the lesser trochanter. Mark the resection line using electrocautery or methylene blue. Resect the femoral head.
Femoral canal initiation
Utilise the modular box osteotome to enter the femoral canal and to establish version. If needed the box osteotome can be used to clear bone laterally.
