Japanese term or phrase: 収入原票
How would one translate this in the context of a utility bill (e.g. water)? Don't know if I'm just missing something here, but I haven't been able to find much information about it in either language.

It appears like this on a water/sewage usage fee payment notice: 納入済通知書(収入原票)

Thanks in advance.

English translation:receipt voucher
Explanation:
収入伝票 means "receipt", and since 原票 is a kind of 伝票 and has seemingly the same meaning as "stub", 収入原票 could probably be translated to "receipt stub", or more generally, "receipt voucher".

Examples of 収入原票 forms can be found at http://www.town.kushiro.lg.jp/reiki/reiki_honbun/a203RG00000...
and
https://www1.g-reiki.net/kutchan/reiki_honbun/a075RG00000668...
The first example appears to be for companies to keep record of customer transactions, with it having spaces to put multiple addresses, as well as corresponding names and money amounts.

Asker: Yes that makes sense, as the document in question is basically a receipt/notice of payment. Thank you for your help!

Asker: It also seems to be a term that only certain prefectures use - this one is also from Hokkaido
