Description of the condition of a patient (who is a construction worker by trade) with a shoulder injury:

"Ajoittain olkapää vaivaa ja kipeytyy, hankaluutta on ylätason työskentelyssä."

Does "ylätaso" here refer to the physical location of the work (e.g. at a great height), or to the quality level (and/or precision level) of the work?
