What I meant was in order to make the thread stay or 'fijarse', that it should be wrapped instead of tied.

alternar la dirección de las vueltas para evitar nudos falsos

Creo Wrap en el contexto que H Blair ha dado se refiere a las vueltas que se le dan a la sutura alrededor del forceps/portaaguja cuando se utilizan intrumentos para anudar.
La técnica consiste en pinzar el segmento libre con el instrumento y enrrollar el otro segmento sobre el cuerpo central del instrumento. El nudo se obtiene al deslizar "las vueltas" de sutura creadas sobre el instrumento sobre el extremo libre de la sutura.
Esta operación se tiene que repetir dos veces por nudo y la segunda vez tienes que enrollar el instrumento en sentido contrario o el nudo se abrirá.

fijar el producto dando vueltas alternas con el hilo con el fin de evitar haciendo nudos

Hay que dar vueltas lo que se supone que es el hilo, para evitar hacer nudos.

Im sure you could make it sound much better in your native language! I hope this helps!

TÉCNICA.


NUDO SIMPLE:
Es el nudo más frecuentemente utilizado en A.P.
•Aguja separada del campo de acción (puede controlarse con el mosquito).
• Enrollar el extremo largo de la sutura (unido a la aguja) alrededor del portagujas con dos vueltas (doble lazada sobre porta, con hilo proximal). Con la punta del porta se sujeta el cabo suelto, y se estiran los extremos para tensar el nudo.
