A proximal box is the shape that the dentist makes in the tooth in order to get access to a cavity that is BETWEEN the teeth (instead of on the chewing or front surfaces, which are easier to get to directly.) The proximal surface of a tooth is that surface that touches the other teeth; the "side" surfaces. (Each side also has a more specific name, mesial or distal.)

So the dentist makes a box with the handpieces down the side of the tooth to get to the decay so that it can be removed. It looks like this:

It's basically just a little box that's removed. Once the decay is gone, a band is put next to the tooth so you have a shape to squish the filling material against, and then it's filled!
