If your document originates from Britain, as I very much suspect it does, then there is no need to conflate different kinds of medical doctors, their qualifications and careers, and introduce gratuitous ambiguity in the translation of this very straighforward service. A general practitioner in Britain is what a family doctor is in Albania. Just for the fun of it, here's my GP's website: https://www.grandunionhealthcentre.co.uk/team/doctors/ . Anyone's free to contact them and ask what their services are for.
My only smithereen of doubt here would be about what exactly the premises are called where a family doctor in Albania runs his practice from.
